To keep things moving with the application process, please remember to submit the following:

Proof of Child’s Age/Identity: Please provide your child’s Birth Certificate. If you do not have one, we can also accept immunization records or a government-issued ID.

Household Income Verification: Please provide pay stubs, W2s, Tax Form 1040 (page 1), or a Notice of Eligibility letter for SNAP, TANF, or SSI.

For Foster or Kinship Care: If the child you applied for is in foster care or is not your biological child, please provide the court paperwork (such as a placement order or temporary custody order). Note: For children in foster care, this paperwork replaces the need for personal household income verification.
Please note that we are unable to process applications until all documents are received.
